Michael Postelnick currently is System Coordinator, Antimicrobial Stewardship and Senior Infectious Diseases Pharmacist at Northwestern Medicine, a multi-hospital system in the Chicago area. He practices at McHenry Hospital, Huntley Hospital and Woodstock Hospital and is board certified in Pharmacotherapy with Added Qualifications in Infectious Diseases. He currently holds adjunct faculty status at Midwestern University College of Pharmacy as well as an appointment as Lecturer in Pharmacology in the Northwestern University Feinberg School of Medicine Department of Medical Education. He has authored over 65 peer-reviewed publications. He has been involved in the education and training of pharmacy students and pharmacy residents for over 30 years. Mr. Postelnick received his Pharmacy degree from the University of Illinois in 1976 and completed a residency in Pharmacy and Pharmacology at Rush University Medical Center in 1980. He became board certified in 2003. In 2008, he received the Outstanding Clinical Practice in Infectious Diseases Pharmacotherapy Award from the Society of Infectious Diseases Pharmacists. Mr. Postelnick has served as Chair of the Society of Infectious Diseases Antimicrobial Stewardship Certificate Committee in 2013 and 2014. He currently serves on the Vizient Pharmacy Council Antimicrobial Stewardship Committee and as Chair of the Vizient Pharmacy Council Research Committee. |

Dr. Jaime Borkowski received a Doctor of Pharmacy degree in 2001 at Butler University in Indianapolis, IN, and completed a Pharmacy Practice Residency in 2002 at Northwestern Memorial Hospital in Chicago, IL. She is currently a Practice Coordinator for the NM Antimicrobial and Diagnostic Stewardship Program (ADSP) and the lead Antimicrobial Stewardship Pharmacist at NM Delnor Hospital, where she has been working the past 15 years to develop the program. Dr. Borkowski serves as a preceptor for clinical rotations for pharmacy residents and students. Muhammad Dhanani, MD

Dr. Muhammad Dhanani is an infectious diseases physician and clinical instructor in the Division of Medicine at Northwestern University's Feinberg School of Medicine. He completed internal medicine residency and infectious disease fellowship at Boston University School of Medicine/Boston Medical Center, where he also completed an advanced fellowship in antibiotic stewardship, hospital epidemiology & infection control. Between residency and fellowship, he worked for two years as a hospitalist at Mount Auburn Hospital, a community-based teaching hospital with an independent internal medicine residency program and an affiliation with Harvard Medical School. During his hospitalist years, he completed a fellowship in health professional education. His medical degree is from University of Michigan Medical School. His interests include development of antibiotic stewardship education materials using established techniques in medical education, and other domains of stewardship such as diagnostic stewardship and vascular access device stewardship. |

Dharmesh is the Clinical Quality Program Manager with Northwestern Medicine, focused on improving care in outpatient settings. The goal of the outpatient quality team is to facilitate safe, effective, patient-centered, timely, efficient, and equitable care. Dharmesh is driven by the principles of process improvement and adheres to a data-driven approach to solving issues in healthcare. Dharmesh has been part of the ambulatory antimicrobial stewardship committee at Northwestern Medicine since 2018 and has been facilitating outpatient committee and related activities. Dharmesh played an intricate role in developing and implementing interventions in the outpatient settings and, more importantly, sharing individual clinicians' prescribing data and comparing that to their peers. Dharmesh received his Master of Business Administration (MBA) degree from Ohio University in 2019, Bachelor of Science in 2015, and Nuclear Medicine Technology, Associate in Applied Science in 2008. Since graduating in 2008 and before his clinical quality role at Northwestern Medicine, Dharmesh has held radiology and healthcare informatics roles at acute care hospitals. With this career journey, Dharmesh is now a healthcare professional with more than 13 years of experience in the industry with excellent skills in complex project management, process improvement, informatics, quality and patient safety, and data analysis. Dharmesh is raising a young family with his wife in Skokie, IL. Dharmesh has two sons, aged six years and one year. |
